Instructions for Families Applying for Financial Aid
For 2011-2012 School Year
- Click on the prompt to begin your PFS. If you have more than one child applying to our school, you only have to complete one PFS. You will be given a password that will allow you to return to your PFS at a later date before submitting it. If you completed an online PFS application last year, use your e-mail address and existing password. If you have forgotten your password, click on “Forgot my password.”
- Gather electronic copies of the following required documents:
• Copies of W-2s for the 2010 tax year.
• Most recent tax returns, including all schedules.
- To submit these documents online, go to the Manage Documents tab in the PFS Online. There you can upload documents from your computer. Save each document separately in one of the following formats: .pdf, Microsoft Word, .jpg or .tif. On the Manage Documents page, click on “Browse,” locate the document, then click on “Upload.”
- To submit these documents by mail, download and complete the Required Documents Cover Sheet. Send copies of your documents – not originals. Do not use staples or paper clips. Mail any documents 10 days in advance of our deadline to allow for mail delivery and for SSS’ processing time.

- Submit and pay for the PFS ($35). This fee is non-refundable.
- Go to STEP 2.

STEP 1 – COMPLETE PFS BY MAIL OPTION
- Download and complete the Parents’ Financial Statement (PFS).
- For Section B of the application, SBMS school code is 904267
- Gather copies of the following required documents:
• Copies of W-2s for the 2010 tax year.
• Most recent tax returns, including all schedules.
- Download and complete the Required Documents Cover Sheet.
- Send copies of your documents – not originals. Do not use staples or paper clips.
- Mail the PFS, Required Documents Cover Sheet, and required documents with a non-refundable payment of $47 to:

STEP 2 – REQUIRED DOCUMENTS
Please complete the SBMS Financial Aid Statement). It is due January 14, 2011 for returning families and February 16, 2011 for applicant families.
